Polo &T-shirt Order Form

  • Central Carolina Chrysanthemum Society members, especially our new members, please think about ordering a CCCS shirt. 

 

  • You will have many opportunities to wear our shirts and share the news about our rare mums. CCCS will be participating in the Ideal Home Show, the NC State Fair, and of course, the 2021 National Chrysanthemum Society Convention and Flower Show in Durham this fall.

  •   These are the same shirts that we ordered in prior years

  • The order form for the polos and T-shirt is attached below.

  •    All orders are prepaid and need to be in by Friday, July 30th

  • Two payment options are available: using this fillable form paying with our PayPal account or mailing in a completed hard copy with a check made out to 'CCCS' addressed to Joan Matthews, 1901 Glenwood Ave, Raleigh, NC 27606

  • All shirts are black with the embroidered CCCS logo

  • All shirts are 100% with minimal shrinkage

  • Tax is included

  • Shirts will be distributed at our September CCCS meeting at JCR Arboretum  or picked up at Joan's garage
